Lesson 4: The Application

Introduction

The application process is a comprehensive first step that involves more than just filling out forms. This lesson will guide you through each component of the application process, including the home study, supporting documents, and required education.

Application Components

Initial Application

  • Schedule a call or a meeting with Lisa Blackham, Adoptive Family Lead, at lisab@hearttoheartadopt.com
  • Complete the online application through the Heart to Heart Adoption website
  • Submit the non-refundable application fee of $2,000 (payable by Venmo or PayPal, or credit card)

Home Study Requirements

A home study, or Adoptive Pre-Placement Evaluation, includes:

  • One or more interviews with a social worker
  • Written documentation about your:
    • Medical background
    • Marriage and family history
    • Parenting styles
    • Financial situation
  • Three reference letters (1 related, 2 non-related)

Home Study Options and Costs

  • For Utah residents:
    • Complete home study: $850
    • Updated home study: $250
    • Post-placement visits: $150 each
  • For non-Utah residents:
    • Must work with a local social worker—normally recommend using a licensed adoption agency
    • Heart to Heart can provide recommendations in some areas
  • With a few exceptions, such as Arizona, home studies are valid for one year. If there are major changes within the household, updating may be required.

Supporting Documentation

Required documents include:

  • Child and Adult Abuse Checks (for all household members 18+)
  • Driver’s License color copy (front and back)
  • Medical insurance card copy
  • Medical clearances for all household members
  • Birth certificates for adoptive parents and children
  • Marriage license and divorce decrees (if applicable)
  • Three reference letters (one from family, two from non-family)
  • Training and education certificates
  • Background Criminal Investigation (BCI) checks for all adults in home

Family Profile Creation

  • Create a profile book showcasing your family
  • Submit a PDF version during application
  • Provide three physical copies
  • View examples on the Waiting Families webpage
  • Complete your profile on the Waiting Family’s page (Indicate if you wish to have it visible.)

Required Education

  • Complete 10-12 hours of adoption-related training
  • Access to Creating a Family resource (included in application fee)
  • At least two hours of reading materials from recommended books such as:
    • “Raising Adopted Children” by Lois Ruskai Melina
    • “Twenty Things Adopted Kids Wish Their Adoptive Parents Knew” by Sherri Eldridge
    • “Real Parents, Real Children” by Holly Van Gulden and Lisa M. Bartels-Rabb
    • “Lifegivers” by James L. Gritter
